TERMS

General

It is the responsibility of the practitioner to contact their regulatory board to determine whether the treatment modalities (including, and not limited to: botulinum toxin, dermal fillers, and/or lasers) taught in the hands-on courses they are registered for are within their scope of practice.

Level 1

Upon registration, practitioners will have access to the online course material for one year. It is the responsibility of each practitioner to ensure they complete the course exam prior to their membership expiration and prior to attending the hands-on portion of the Level 2 course. Practitioners may register for a Level 2 course prior to completing the Level 1 exam.

Patients for Hands-on Courses


Each practitioner is required to provide a patient of record as a patient model for each of the hands-on sessions in Level 2, Level 3, Level 4 and Level 5. If unable to do so, it is the responsibility of the practitioner to notify PTIFA at [email protected] at least four weeks prior to the hands-on date. This is to ensure PTIFA can maximize the amount of hands-on experience each practitioner has at the program.

Practitioners understand they are responsible for their patient’s treatment cost at the program. For fee details, please contact us.

Practitioners are responsible for providing a copy of their current license and malpractice insurance to PTIFA, prior to attending each hands-on course. Each practitioner is responsible for providing follow-up care for their patient model after the course.

Registration & Cancellation Policy


PTIFA dedicates a significant amount of resources and makes a number of commitments when scheduling courses. For this reason, all payments and registrations are non-refundable and non-transferable.

Level 2, Level 3 and Level 4 hands-on course date changes are not permitted within 60 days prior to the course date. Changes made prior to 60 days before the course date are subject to a $500 rescheduling fee.

All deposits are non-transferable. A deposit will be taken at time of registration, with the balance due 60 days prior to the course date.

PTIFA reserves the right to cancel, post-pone or change any course without notice.

Partners

PTIFA has partnered with a number of organizations in order to provide you with the best learning experience and ability to improve patient care. PTIFA does not have any vested interest in our partner’s products, nor does any of our faculty members. Practitioners understand that their name, email address and phone number may be shared with PTIFA’s partners after attending the program. If they wish to not be contacted, they will have the chance to opt-out at the program.

License Verification

It is the responsibility of the practitioner to provide a valid and active license prior to attending a hands-on program. PTIFA will verify practitioner’s appropriate license information.

Travel & Accommodation

PTIFA is not responsible for hotel, airfare or other expenses incurred to attend the program.

PTIFA is not responsible for hotel, airfare or other expenses incurred due to course changes/cancellations.

TERMS OF USE

This Privacy Policy governs the manner in which Pacific Training Institute for Facial Aesthetics collects, uses, maintains and discloses information collected from users (each, a “User”) of the https://ptifa.com website (“Site”). This privacy policy applies to the Site and all products and services offered by Pacific Training Institute.

We may collect personal identification information from Users in a variety of ways, including, but not limited to, when Users visit our site, register on the site, subscribe to the newsletter, fill out a form, and in connection with other activities, services, features or resources we make available on our Site. Users may be asked for, as appropriate, name, email address, phone number. Users may, however, visit our Site anonymously. We will collect personal identification information from Users only if they voluntarily submit such information to us. Users can always refuse to supply personally identification information, except that it may prevent them from engaging in certain Site related activities.
